Once you complete and submit the online referral form, we'll immediately receive it and contact your patient to figure out a time that works best for them. When an appointment is confirmed, we'll notify your office. If you would prefer us to contact your office instead of your patient, or they're in need of an emergency appointment, please note it in the comment section of the form.
